What this site collects. The partnership application form collects your name, email, phone if you provide it, business name, website or social handle, home state, business type, audience size, program interests and any notes you write. We use this only to respond to your enquiry and evaluate a business partnership. It is delivered by email to ConduitMD and is not sold or shared with third parties for marketing.

Health information. This website is a business-to-business site. It does not collect, and you must not submit, any patient or personal health information. Patients of a partner program are served by an independent medical practice under that practice's own privacy notice.

Analytics and cookies. This site may use privacy-respecting, aggregate analytics to understand which pages are visited. It does not use advertising trackers.

Your rights. To access, correct or delete anything you have submitted, email partners@getconduitmd.com.

Changes. Updates to this policy are posted here with a new effective date.
